General Description
Â
Specific responsibilities include, but are not limited to:
·     Scheduling weekly medical, dental, and M/H appointments, preparing for scheduled court hearing, and accompanying clients to family support conferences and other CUA related activities.Â
Â
·     Determining weekend pass list for children, restriction list based on disciplinary write ups, and communicating incident and HCSIS report with contracting agencies.
Â
·     Reviewing electronic files and verification forms for completed medical and dental appointments weekly.
Â
·     Assessing master client list, active client, and discharged client charts weekly.
 Â
·     Conducting client intakes and completing intake packets.
Â
·     Assisting with employee evaluations and arranging personnel training in compliance with 3800 regulations.
Â
·     Arranging and identifying clients to participate in weekly Flow of Service meetings and other case consultations.
Â
·     Scheduling or un-scheduling events or external agency activities attended.
Â
·     Developing and maintaining marketing contacts with outside agencies and referral sources.
Â
Social Service specific responsibilities include:
·     Meeting monthly with every child and documenting interaction with child, bio parent, or reunification resource.
Â
·     Maintaining contact with CUA Case Manager, Legal Parties, etc.
Â
·     Attending family teaming conferences and court hearings with children. Identifying subcontractor goals and ALANA MILANS PLACE specific DRO orders.
Â
·     Following up with family findings and connections, therapy (group and individual), mentoring, and life skills. Evaluating process for discharge/DRO review.

Â
·     Communicating with Bio parents (phone, written communication, and home visits).
Â
·     Discharge planning- continuum of therapeutic needs, family life, secure educational needs, ensure safety (home visit), identify permanency resource.
Â
·     Permanency planning in relationship to goal: Reunification, Adoption, PLC, Fit/Willing relative, APPLA.
Â
Family Engagement: Bio parent/Reunification Resource
·     Minimum of 2 contacts a month with bio parent or reunification resource.
Â
·     Progress reports- written documentation of telephone contacts and other outreach efforts with reunification resource.
Â
·     ISP goals written by therapeutic staff at ALANA MILANS PLACE- mailed home to family members.
Â
·     Invite family members/reunification resource to conference (20 day, 90 day, placement stability conference.
Â
·     Reunification plan- connected with DRO goals, single case plan, and treatment plan. Once objective/goals are obtained, family should be made aware of potential reunification dates (communicated and agreed upon by all parties including treatment team and legal including child advocate, ACS, etc).

·     Monitor/supervise family visits at ALANA MILANS PLACE location
Â
Child Engagement:
·     2 face to face documented contacts per month.
Â
·     Invite and accompany child to all conferences.
Â
·     SWAN application follow up and interviews with affiliates.
Â
·     Discuss/review single case plans and document interaction with children during face to face visits.
Â
·     Review child safety plan during face to face engagement with children.
Â
·     Refer activities- documented outreach/ referrals in charts, linked to Ansell Casey life skills assessment. Site manger will be able understand cultural competency and make referrals based on motivational interviewing. Strength based referrals.

·     Financial budget/plan-discuss allowances, savings, and budgeting.
Â
·     Discuss daily activities and schedules. Discuss time management in relationship to stated goals and objective
Â
Qualifications
Â
A Site Manager shall have the following:
Â
·      Must have a Master’s degree from an accredited college, or university, and one year of work experience with children or youth.
Â
·      All staff will be required to participate in prescribed annual training.
Â
·      An associate’s degree or 60 credit hours from an accredited college or university, and 3 years work experience with children or youth.
Â
·      Must be able to work rotating shifts, and the ability to arrive in emergencies within 30 minutes.
Â
·      Must have reliable transportation.
Â
·      Bi-lingual is a plus.
Â
Persons in this position are required to maintain licenses in good standing, if applicable. All candidates must pass a Child Abuse Clearance and Pennsylvania Criminal Record search, plus an FBI criminal history clearance if resident outside Pennsylvania. All staff will be required to participate in prescribed annual training.
